AUDIENCE
This program is designed for those health-care providers involved in routine care of patients with diabetes including:
Endocrinologists • Primary Care Physicians • Nurse Practitioners • Clinical Nurses Physician Assistants & Other Members of the Diabetes Care Team

OBJECTIVES
At the conclusion of this activity, participants should be able to: Recognize the future directions of diabetes care in order to provide the best care for patients today
Explain how to provide individualized, patient-centered care to patients with diabetes to manage risks, improve outcomes and achieve positive patient experience
Demonstrate value of each member of the patient care team to individualize care, improve outcomes and maximize patient experience
Review current safety, efficacy and clinical trial data to make informed evidence-based treatment choices for patients with diabetes
